SMS Appointment Reminder Terms and Conditions

Effective date: March 1, 2026
Last updated: September 6, 2026

These SMS Appointment Reminder Terms and Conditions (“SMS Terms”) govern the text-messaging program operated by Blossom Speech and Language Therapy TN (“Blossom,” “we,” “us,” or “our”).

1. Program Description

Blossom offers an optional text-messaging program for patients and their authorized parents, guardians, or representatives. Messages are limited to non-marketing communications related to scheduled services, including:

  • Appointment reminders

  • Appointment confirmations

  • Scheduling and rescheduling requests

  • Appointment cancellations or changes

  • Clinic closure or delay notifications

  • Responses to appointment-related questions

  • Other administrative communications directly related to scheduled services

Enrollment in this program does not authorize Blossom to send marketing or promotional messages. Separate consent will be obtained before sending messages for any materially different purpose when required.

2. Consent to Receive Text Messages

By providing your mobile phone number, actively selecting the SMS consent checkbox, and submitting the enrollment form, you expressly consent to receive recurring SMS text messages from Blossom at the mobile number you provided.

Your consent applies only to the appointment-related and administrative communications described in these SMS Terms. Your consent is specific to Blossom and may not be sold, rented, transferred, or used by another business to send you messages.

You confirm that:

  • You are the subscriber or customary user of the mobile number provided, or you are otherwise authorized to provide consent for that number.

  • The information you provide is accurate.

  • You will notify Blossom if your mobile number changes or is reassigned.

  • If you are enrolling on behalf of a patient, you are authorized to receive communications concerning that patient’s appointments.

Consent to receive text messages is optional and is not a condition of purchasing, receiving, or continuing treatment or other services from Blossom.

3. Message Frequency

Message frequency varies based on the number of scheduled appointments, appointment changes, and your communications with Blossom.

You will typically receive approximately 1–3 messages per scheduled appointment. Additional messages may be exchanged when needed to coordinate scheduling, respond to your request, or communicate a closure, delay, cancellation, or other appointment-related change.

4. Message and Data Rates

Message and data rates may apply according to the terms of your mobile carrier and service plan. Blossom does not charge a separate fee for participation in the SMS program, but your carrier may impose messaging, data, or other charges.

5. Opting Out

You may opt out of the SMS program at any time by replying STOP to any message from Blossom.

After submitting a STOP request, you may receive one final text message confirming that you have been unsubscribed. No additional SMS messages will be sent unless you later provide consent again.

You may also withdraw your consent by contacting Blossom at:

Phone: 615-707-4644
Email: contactustn@blossomslt.com

Withdrawing SMS consent will not affect your access to treatment or other services. You remain responsible for maintaining appointment information through another available communication method.

To resume receiving messages after opting out, reply START or contact Blossom and provide consent again.

6. Help and Customer Support

For assistance with the SMS program, reply HELP to any message.

7. Mobile Carrier Disclaimer

Mobile carriers are not liable for delayed or undelivered messages. Message delivery is subject to the availability and operation of wireless networks and services and is not guaranteed.

The SMS program may not be available through every mobile carrier or on every mobile device. Blossom is not responsible for service interruptions, transmission failures, delayed messages, misdirected messages, or other circumstances outside its reasonable control.

8. Privacy

Your participation in the SMS program is also governed by our SMS Privacy Policy, which explains how we collect, use, protect, and disclose information associated with text messaging.

Mobile phone numbers, SMS opt-in data, and text-messaging consent will not be sold, rented, or shared with third parties or affiliates for their marketing or promotional purposes.

9. Security and Sensitive Information

Text messages may not be encrypted and may be viewed by anyone who has access to your mobile device or mobile account. By enrolling, you acknowledge these risks and authorize Blossom to send the appointment-related and administrative messages described in these SMS Terms.

Blossom limits messages to information reasonably necessary for appointment-related communications. Please do not send detailed medical information, financial information, Social Security numbers, passwords, or other highly sensitive information by text.

10. Emergencies and Medical Advice

The SMS program is provided only for appointment-related and administrative communications. It is not intended for emergencies, urgent medical concerns, diagnosis, treatment advice, or other time-sensitive healthcare needs.

Do not use text messaging to request emergency assistance. If you are experiencing an emergency, call 911 or seek appropriate emergency care.

11. Termination or Modification of the Program

Blossom may suspend, modify, or discontinue the SMS program at any time. We may also suspend or terminate participation when we reasonably believe a mobile number has been reassigned, consent is no longer valid, messages cannot be delivered, or continued messaging would violate applicable law, carrier requirements, or these SMS Terms.

12. Changes to These Terms

We may update these SMS Terms periodically. Any revised version will be posted on our website with an updated revision date. Changes will apply prospectively unless otherwise required by law.

Continued participation after an update constitutes acceptance of the revised SMS Terms to the extent permitted by applicable law. When required, we will obtain renewed consent before materially changing the purpose or subject matter of the messages.
